PGY1 & PGY2 Application

There are two acceptable methods for interested resident candidates to apply for either the PGY1 Pharmacy Residency or the PGY2 Critical Care Residency Programs:

Method #1

You may choose to apply DIRECTLY to the Saint Joseph Hospital Residency Program WITHOUT using the PhORCAS.  There is no fee associated with applying by this method.

Instructions for submitting application materials by this method:

 

1.    Download the Saint Joseph Hospital Residency Program Application here: Residency Program Application

  • Note:  The Residency Program Application form may be used to apply for either the PGY1 Residency or PGY2 Critical Care Residency Program.


2.    Email the following items to the appropriate Residency Program Director using the email contacts listed on the Residency Program Application:

  • Personal Cover Letter
  • Curriculum Vitae
  • Completed Saint Joseph Hospital Residency Program Application


3.    Download our Recommendation Cover Form here:  Recommendation Cover Form

  • Provide this document electronically to THREE professional references of your choice.  This document contains instructions for how references may email their recommendations directly to the appropriate Residency Program Director.


4.    Mail a hard copy of your Official College Transcripts to the appropriate Residency Program Director.

Method #2

You may choose to use ASHP’s Pharmacy Online Residency Centralized Application Service (PhORCAS).

Instructions for submitting application materials by this method:

1.    Applicants choosing to apply through PhORCAS must download the Saint Joseph Hospital Residency Program Application here:  Residency Program Application

  • Note:  The Residency Program Application form may be used to apply for either the PGY1 Residency or PGY2 Critical Care Residency Program.


2.    Applicants can then upload their completed Saint Joseph Hospital Residency Program Application, Personal Cover Letter, and Curriculum Vitae to the PhORCAS website.

3.    Request THREE professional references of your choice to complete recommendations for you through PhORCAS.

4.    Provide your Official College Transcript materials to the PhORCAS service.  PhORCAS will then make your transcripts available to the Residency Program Director along with your other application materials.  
 
All application materials must be received by the application deadline:  January 2nd

Resident applicants will be notified of their status for on-site interviews by January 31st.
