KentuckyOne Health Partners adds 10 skilled nursing facilities in Lexington

Lexington, Ky. (April 21, 2015) - KentuckyOne Health Partners, part of KentuckyOne Health, is making strides to provide more accountable, coordinated care for those in need of skilled nursing care in Lexington. The organization has added 10 skilled nursing facilities to the KentuckyOne Health Partners network, the largest accountable care organization in the state and a clinically integrated network. A meeting will be held April 22, 2015 to introduce the consortium of skilled nursing facilities in Lexington.

Skilled nursing facilities added to the network include: Bluegrass Care & Rehab, Cardinal Hill Rehabilitation, Danville Centre for Health and Rehabilitation, Harrodsburg Health & Rehab, Mayfair Manor, Northpoint/Lexington Healthcare Center, Providence Homestead Nursing Care, Richmond Place Rehabilitation & Health Center, Signature Healthcare of Georgetown and Tanbark Health & Rehabilitation Center.

“These new partnerships allow our staff to work collaboratively with the patient, physician, patient navigator and other caregivers to ensure patients are getting the right care when and where they need it,” said Damian “Pat” Alagia, MD, chief medical officer, KentuckyOne Health. “Partnerships like this are another way that we're able to provide wellness, healing and hope to all, including the underserved.”

The goal of these partnerships is to improve the continuum of care, providing better care and patient experience at a lower cost.  At the same time they reduce hospital readmissions, increase satisfaction and provide the right care at the right time and in the right place. The partnership will improve transition of care and simplify documentation.

It will also offer opportunities for more patient and family engagement, by making sure the patient is involved in decision-making about his or her care.  The partnership will employ sophisticated information systems so that everyone involved in a patient's care can see the same information, which allows for better clinical decisions and eliminates duplicate services or medications.

KentuckyOne Health Partners is also adding skilled nursing partnerships in the Louisville area.
