KENTUCKYONE AND HEALTHCARE CLINIC AT SELECT WALGREENS COLLABORATE TO IMPROVE PATIENT HEALTH THROUGH CONTINUITY OF CARE IN LOUISVILLE AND SOUTHERN INDIANA
Collaboration improves patient care by coordinating care plans and records between providers

 

Louisville, Ky. (July 14, 2014) – KentuckyOne Health collaborates with Healthcare Clinics at select Walgreens (NYSE: WAG) (Nasdaq: WAG) to provide greater continuity of care for patients. The clinical collaboration between KentuckyOne Health and Walgreens, originally announced in March, expands coordinated health care services and improves access to high-quality, convenient and affordable care for patients in the Louisville and Southern Indiana area.

 

The KentuckyOne Health and Walgreens care collaboration includes patient access to Healthcare Clinic nurse practitioners and physician assistants as a walk-in care option for minor injuries or illnesses, health testing and screening and support when physicians’ offices may be closed, including after-hours and weekends, and an Emergency Room (ER) visit is not required.

 

KentuckyOne Health physicians are in direct communication with Healthcare Clinic nurse practitioners and physician assistants to facilitate care coordination and sharing of patient information.

 

Patients treated at the Healthcare Clinics at select Walgreens can be referred to a KentuckyOne Health physician for needed follow up care. With patient consent, Walgreens will send a record of the visit to the KentuckyOne Health primary care provider to help keep medical records complete. A complete medical record can help physicians identify potential health risks before they become more serious, which is an important part of staying well and managing chronic diseases such as diabetes.

 

While Healthcare Clinics at select Walgreens and KentuckyOne Express Care strongly encourage all patients to have a designated primary care physician and medical home for ongoing needs, the collaboration will provide patients access to quality care seven days a week, even when their primary care physician is not available.
