Clinic and Hospital Patient Portal
The patient portal invitation you receive after visiting one of our facilities will never expire. Once you sign up, My OneCare will be available to you 24/7 with anytime access via computer, smartphone or tablet.
Please view our Patient Portal Frequently Asked Questions for more information. If you are a representative (proxy) for your child or a dependent adult and need to access health information to make informed decisions, review our Patient Proxy or Power of Attorney guide.

You can also use another application to access your health information, if you prefer. To do so, you will need to first set up a portal account with My OneCare. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.

MyCancerCare offers you tools and resources to make it easier to manage your health and the health of your loved ones. It keeps your health information at your fingertips. MyCancerCare is not for urgent or emergency requests. If you have an immediate health need or your request needs an answer right away, please call your provider’s office or hospital.
