My OneCare is not to be used for urgent medical matters. If you need urgent medical assistance please call 911 or your doctor's office.

To access the My OneCare Clinic Patient Portal please click here. This will include all physician practices and ambulatory facilities, e.g; CHI Saint Joseph Medical Group - Primary Care, CHI Saint Joseph Medical Group Cardiology, etc. For frequently asked questions please click here.

The patient portal invitation you receive after visiting one of our facilities will never expire. Once you sign up, the portal will be available to you 24/7.

You can also use another application to access your health information, if you prefer. To do so, you will need to first set up a portal account with My OneCare. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.

MyCancerCare offers you tools and resources to make it easier to manage your health and the health of your loved ones. It keeps your health information at your fingertips. MyCancerCare is not for urgent or emergency requests. If you have an immediate health need or your request needs an answer right away, please call your doctor’s office or hospital. It may take up to 2 business days to receive a response via MyCancerCare.
