Pharmacy Refills

The most efficient approach for requesting refills is to phone your pharmacy directly and have them contact our office by fax.

Your pharmacy will send all necessary information to us. If you are absolutely unable to contact your pharmacy for a prescription refill please have the following information ready when you call:

• Name and correct spelling of drug
• Current dose
• Pharmacy name and phone number

When requesting refills from mail-in pharmacies please request a written prescription or signature on the form provided to you by the mail-in pharmacy. Make sure we have your current and correct ID information.
