Video Production Request

This video request form will help Media Services understand and fulfill your video production objectives and creative needs.  Please submit request at least 3 weeks prior to your scheduled deadline.  This video request form is for requesting services and does not guarantee the availability for your project.  Due to volume of requests, some requests may have to be handled by an outside approved CHI vendor.  In those instances, Media Services can supply you with a list of these vendors.  The Requestor will accept responsibility in facilitating the production process and billing charges associated with outside vendor, unless otherwise noted.

Points To Consider:

• Please review each of these questions to help determine production objectives.
• Who is the audience and who will see the finished program?
• What information or impression should they bring away from the project?
• What are the key/learning points and key information to be covered?
• What is your availability during the process? (You need to allow time for meetings, organizing aspects of production, to be present at videotaping, and review of finished product.)
• Will a professional script be needed?
• What is the estimated production time?
• What types of on-camera talent will be needed (interviews, actors, B-Roll, public, etc.) and are all talent usage release forms signed before at time of production?
• Do you have/need location shooting permission/clearance and are all locations reserved for the scheduled time of production?
